Often, youth who are transitioning from the Village House residential program receive wraparound services for a period of time as they adjust to living on their own in the community. The Village House Wraparound program is designed to support these transition age youth by providing case management and supportive counseling on either a weekly or bi-weekly basis, depending on what the youth's needs are. The Village House Wraparound team helps the youth navigate their community and adjust to living on their own and to adulthood responsibilities.
Village House Wraparound can serve up to 3 transition age youth. They are provided with weekly face-to-face contact to ensure they are meeting their needs and to address any concerns they may have in their work place, school, community organizations, medical facilities, and other entities they come in contact with. They are encouraged to contact their service provider at any time and are provided with supportive counseling, encouragement, and collaborative access to community resources. Duration of services is individualized but may be anywhere between two to six months.
