Serving children, youth, and their families in their homes and communities enhances opportunities for client and family engagement and collaboration within their local provider network.

Wraparound Programs

Wraparound is an intensive, individualized care management process for youths with complex emotional, behavioral needs, and multiple agency involvement. Clients/families benefit from a coordinated care planning process that produces a single plan of care that engages other agencies and providers. The wraparound process strives to ensure children and youth grow up in their homes and communities. In an effort to support the client/family goals, clinical case managers help to coordinate activities, formal interventions, community services and interpersonal support and assistance provided by friends, kin, and other people drawn from the family’s social networks.

The 10 Principles of the Wraparound Process:

  • Family voice and choice
  • Team based
  • Natural supports
  • Community based
  • Collaboration
  • Cultural competence
  • Individualized
  • Strength based
  • Persistence
  • Outcome based
